Ongoing Support at Sereno Clinic

Recovery and mental wellness are not destinations, but continuous journeys. At Sereno Clinic, our Ongoing Support services are designed to provide the long-term stability and clinical partnership you need to thrive after your initial treatment goals are met.

We bridge the gap between intensive treatment and independent living by offering a structured safety net that evolves with your needs.

A Foundation for Long-Term Success

Our Ongoing Support program is a core pillar of our Outpatient Psychiatric Care services, ensuring you are never navigating your mental health journey alone. This includes:

  • Continuous Clinical Monitoring: Regular check-ins to assess your well-being, celebrate progress, and identify challenges early.
  • Relapse Prevention: Development of personalized wellness strategies and early-warning systems to maintain progress.
  • Adaptive Care Planning: Adjusting your treatment plan as your life circumstances evolve.
  • Resource Coordination: Guidance in accessing external support systems, community resources, and lifestyle improvements.

Stability Within the Outpatient Care Framework

Ongoing Support works alongside our other outpatient services to create a comprehensive care experience:

  • Medication Consistency: Collaboration with Medication Management to ensure prescriptions remain effective and appropriate.
  • Flexible Access: Availability through Telehealth Visits for convenient, consistent care.
  • Holistic Integration: Alignment with psychotherapy goals to support overall mental health progress.
